Cosmetic Skin Rejuvenation

Skin rejuvenation and resurfacing can be achieved in a number of ways, ranging from laser, light and other energy-based treatments to chemical peels and other non-ablative methods.Skin rejuvenation is a treatment to resurface your skin, or to improve the texture, clarity and overall appearance of your skin.

Conditions which can be treated include:

Static Wrinkles:

These wrinkles are visible at all times and do not change in appearance with facial movements

Dynamic wrinkles:

These are expression lines that may appear as folds when the skin is not moving, and deepen with facial movements or expressions

Freckles, sun spots, melasma, or other darkened patches of skin result mainly from sun exposure

As the result of acne or injury to the skin, scars may be rolling (a wavy appearance to the skin), pitted, discolored, or have raised borders

Vascular conditions:

Blood vessels visible on the surface of the skin, vascular lesions that appear as tiny blood filled blisters or even a constant flush of facial redness

Loss of skin tone:

Weakening of the supportive skin structure (collagen and elastin fibers) that results in a loss of skin firmness, or the development of cellulite

Blepharoplasty (Eyelid Surgery)

Eyelid surgery, or blepharoplasty, corrects sagging or drooping eyelids, as well as bags and bulges around the eyes. These conditions develop as a result of laxness of the eyelid skin and accumulation of fat around the eyes. It is not limited to older patients, as many individuals inherit a predisposition toward development of fat pouches at an early age. Thus, surgical correction is appropriate for all age groups.

Following the natural lines of your eyelids, the surgeon makes incisions using a scalpel or laser (see Laser Treatments / Surgery). Excess skin can then be removed by traditional excision or by laser; nonessential fat is removed precisely (to avoid a future "hollowing" appearance as the patient increases in age). Very fine sutures are used to close the incisions (and can be removed in two to seven days).

Following surgery, to help speed their recovery, patients are asked to apply ice compresses during the first 24 to 48 hours, and antibiotic drops for a week. With the reduction of swelling, most patients are able to return to their normal activities in less than a week.

In some cases, abnormal eyelid conditions can be caused by other medical problems such as allergies or thyroid disorders. In these patients, the proper treatment may be medical rather than surgical.

Cheek & Chin Implants

Cheek implants (malar augmentation) and chin implants (mentoplasty) modify the underlying structure of the face to produce a pleasing combination of facial elements. Using components of classic beauty, in this case prominent cheekbones and a strong chin projection, Dr. Michelson creates a countenance of balance and distinction.

The proportioning qualities of these procedures can benefit patients in many ways. Both can enhance a facelift, correct asymmetries and congenital defects, cheek implants can recontour sagging mid-facial tissues, and a chin implant can shorten the perceived length of the nose.

Cheek and chin implants are safe, and risk of patient allergic reactions is very low. Non-toxic materials include solid silicone, porous polyethylene, and expanded polytetrafluoroethylene, and all can be custom trimmed by Dr. Michelson.

Malar augmentation and mentoplasty each require thirty to sixty minutes to complete, and can be performed with local anesthesia combined with a sedative (with the patient awake). However, general anesthesia (with the patient asleep) may be recommended. For each implant, a small incision is made, usually inside the mouth (for mentoplasty, inside the mouth or underneath the chin); a pocket to receive the implant is surgically formed; a sterilized implant of the appropriate size and shape is placed in the pocket and secured; the incision is closed using tiny sutures. External sutures can be removed in five to seven days while intraoral sutures will dissolve in about ten days.

Following the procedures, some discomfort and aching can be expected, and can be controlled with medications prescribed by Dr. Michelson. Swelling and some bruising occur, and usually subside within the first two weeks; during the first twenty-four hours, the swelling can be minimized by applying cold compresses while keeping the head elevated. After mentoplasty, an elastic dressing will be in place for a few days. For both procedures, instructions for temporary changes in oral hygiene and eating (a soft food diet) will be provided - the ability to move your mouth will be diminished right after the procedure, and chewing may be uncomfortable for a week to ten days.

Though it may be best to avoid smiling immediately after your surgery, it will be difficult not to smile when your recovery is complete. Your facial features in balance, you can emerge with self-confidence and distinction. We invite you to call today for your personal consultation

Face & Neck Lift

FaceLift in Oxnard, or a rhytidectomy, is designed to restore a more youthful appearance by removing major wrinkles and sagging skin. Children and young adults have smooth, round faces due to evenly distributed fatty tissues in facial contours. As a result of aging, sun exposure and genetics, the facial skin may sag and wrinkle, and folds of the chin, jaw line and neck may become prominent. Facelift surgery helps to temporarily reverse this process by tightening the facial and neck muscles and removing any excess skin. A face lift is frequently performed in conjunction with blepharoplasty (eyelid surgery), a forehead lift, and chin augmentation (and/or liposuction) to create an overall rejuvenation of the face. A single session often achieves the desired results.

Facelifts are performed in outpatient surgical centers or in an accredited physician's office. Local anesthesia with intravenous sedation is most commonly used, with premedication to help you relax. General anesthesia is generally not required but can be used if desired. The surgeon will make incisions inside the hairline at the temple, in front of the ear, then around the earlobe and behind the ear, ending at the scalp area. Excess skin is removed and sagging muscles and connective tissues are tightened. In some cases, fatty deposits are removed from beneath the chin and neck. Incisions are closed with small sutures. The procedure may take up to five hours, depending on whether any additional procedures are included in the operation.

In the weeks following the surgery, the facial skin and area around the ear may be numb. There may also be some temporary discoloration as healing begins, which may take a few months to disappear. Occasionally, the facial nerve is bruised during surgery, resulting in temporary loss of function or sensation. Serious or permanent damage to the facial nerve is rare. The healing process is gradual, so you will not notice final results for up to six months. Most patients who have had facelifts are pleased with their new, more youthful appearance.

The muscles above and between the eyebrows contract and tighten, and can cause wrinkles. Dysport blocks the nerve signal that causes these facial muscles to contract. This ultimately results in a localized reduction of muscle activity in the treated area and temporarily stops the contraction of these muscles.

What is automated Micro-Needling?

Automated Micro-Needling (also known as Collagen Induction Therapy or CIT) is a new innovation in aesthetic medicine for the treatment of fine lines, acne scars and improvement of the skin's texture, tone and color. During this procedure, tiny needles are used to create controlled micro-injuries to the skin in order to produce collagen and elastin. The skin's repair process results in a thicker dermis with softer wr inkles.Eclipse MicroPen"' also creates micro channels which allow topical gels, creams and serums to be absorbed more effectively, enhancing their effects in deeper layers of the skin.

"I look forward to every treatment because I see results immediately. All of my friends compliment me on my skin now and Iam so happy. Ihave had many other treatments before to treat my Acne scars, and nothing worked as well as the MicroPen . I would highly recommend it to anyone!"

- Elizabeth K.

What are the benefits?

Wrinkle reduction, texture, tone improvement and softer, younger-looking skin with fewer signs of aging. New collagen production reverses years of sun damage. Eclipse MicroPenTM also  has  a  positive  effect  on hyper-pigmentation as well as treating uneven skin tone, acne scars, traumatic scars and stretch marks.

What areas of the body can I treat?

Eclipse MicroPen™ can be used on all parts of the body; Face, neck, decollete, arms, hands, legs, abdomen and back.

How is the procedure done?

A sterile needle-tip containing 12 tiny micro-needles is at tached to the Eclipse MicroPen™.  Your skincare professional will apply a thin layer of topical gel to your skin and select the appropriate treatment depth based on your unique needs. Next, in a single motion, the pen will be gently pressed against the skin while simultaneously gliding in one direction until the entire treatment area has been covered.

What does it feel like?

The feeling associated with the Eclipse MicroPen™ is similar to sandpaper being moved across the skin. While some areas are more sensitive than others, the speed of the reciprocating  needle tip makes the treatment  comfortable.

How long does the procedure take?

The procedure takes 15-30 minutes depending on the size of the area treated.

What is the downtime?

Immediately after the treatment you will notice a bright redness to the skin.  The total healing time depends on the depth of needles used and the number of overlapping passes your skincare professional performs. On average, patients are red for 2-4 days. Some patients heal completely in 24 hours. Utilizing the Eclipse Dermal Recovery System™ post treatment will speed up the healing process.

When will I see the results?

Patients notice an immediate "glow" to their skin. Visible changes to the skin develop over the course of several weeks. Results continue to improve up to 6 months after the treatment as collagen production continues.

How many treatments will I need?

Some patients only require a single treatment, once per year to achieve optimal results. However, it is recommended for most patients to receive a series of 2-3 treatments spaced about 6-8 weeks apart. For patients with deep wrinkles, advanced photo-aging, stretch marks or acne scars it is recommended to receive 6-8 sessions at 6 week intervals.

NovaThread® Lift

The are hypodermic needles preloaded with a PDO absorbable suture.

It’s a synthetic absorbable surgical suture composed of polydioxanone. PDO sutures are used to perform cardiothoracic surgery (such as open-heart surgery). It’s one of the safest materials to be implanted in the body.

In medical terms "Selective Inflammatory Response", it is the principle behind a lot of aesthetic procedures (such as Fractional Laser, Chemical Peels & MicroNeedling). Our skin is very good at repairing itself, and by inserting NOVATHREADS we make it ‘heal’ itself by creating new collagen, naturally!

It's amazingly simple, actually!

Your skin fully absorbs it within 4 to 6 months & won’t leave you with any scar tissue.

Your skin fully absorbs it within 4 to 6 months & won’t leave you with any scar tissue.

Your skin gently reacts to the inserted threads

Yes, PDO is fully absorbable, but that doesn’t mean your skin won’t react to it. During the 4 to 6 months needed to fully absorb the PDO suture, your skin is also repairing itself around it.

It’s "controlled healing" happening underneath the skin!

The NovaThreads are needles that are pre-loaded with a PDO thread. Simply insert the whole needle in the interstitial tissue at subdermal level, parallel to the surface of the skin, & pull the needle out.

The PDO thread remains under the skin.

Although NovaThread® can be inserted almost anywhere, one area where they are especially effective is the neck and jawline.

The lower third of the face is challenging to treat effectively with botulinum toxin or injectable fillers, which are most effective in the upper and middle third of the face, respectively.

Otoplasty (Ear Surgery)

Probably no other physical characteristic cries out for facial plastic surgery more than protruding ears. Children, long the victims of cruel nicknames like “Dumbo” or “Mickey Mouse,” are the most likely candidates for otoplasty, but this surgery can be performed at any age after the ears have reached full size, usually around five to six years of age. Even if the ears are only mildly distorted, the condition can lead to self-consciousness and poor adaptation to school. It is often desirable in children to consider this surgery before they start Kindergarten or first grade

“My ears always stuck out, one more than the other. Growing up, I was very self-conscious. I never wore my hair in a pony tail. It was awful when I went swimming because when my hair got wet, my ears would stick through it. I hated the way they looked. Now that I’ve had surgery, I definitely feel better about my appearance.”

– Joanne J., age 43

Understanding the Surgery

Surgery begins with an incision just behind the ear, in the natural fold where the ear is joined to the head. The surgeon will then remove the necessary amounts of cartilage and skin required to achieve the right effect. In some cases, the surgeon will trim the cartilage, shaping it into a more desirable form and then pin the cartilage back with permanent sutures to secure the cartilage.

In other instances, the surgeon will not remove any cartilage at all, using stitches to hold the cartilage permanently in place. After sculpting the cartilage to the desired shape, the surgeon will apply sutures to anchor the ear until healing occurs to hold the ear in the desired position.

A change in shape will not affect hearing. Although the ears’ folds and convolutions do serve to concentrate and localize sound waves, routine surgery to pin back or reshape ears will not produce a noticeable change in hearing. Ear surgery can offer a real emotional boost, in addition to its cosmetic benefits.

What to Expect After the Surgery

Soft dressings applied to the ears will remain for a few days. Most patients experience some mild discomfort. If you are accustomed to sleeping on your side, your sleep patterns may be disrupted for a week or so because you cannot put any pressure on the ear areas. Headbands are sometimes recommended to hold the ears in the desired position for two weeks after the surgery.

The risks are fairly minimal, but include very rare infections, poor healing or possibility of recurrence of the deformity. Normally, there will only be a thin white scar behind the ear after healing. Because this scar is in a natural crease behind the ear, the problem of visibility is inconsequential. Anything unusual should be reported to the surgeon immediately.

Facial plastic surgery makes it possible to correct many facial flaws that can often undermine one’s self-confidence. By changing how you look, cosmetic surgery can help change how you feel about yourself.

Insurance does not generally cover surgery that is purely for cosmetic reasons. Surgery to correct or improve birth defects or traumatic injuries may be reimbursable in whole or in part. It is the patient’s responsibility to check with the insurance carrier for information on the degree of coverage.

Platelet-rich plasma (PRP)

Platelet-rich plasma (PRP) is blood plasma that has been enriched with platelets. As a concentrated source of autologous platelets, PRP contains (and releases through degranulation) several different growth factors and other cytokines that stimulate healing of bone and soft tissue.

Blood-Derived Growth Factors
The internal essence of your blood contains life-giving fundamental protein growth factors. PRP utilizes a leading-edge patented technology specifically designed. With a simple, quick and advantageous process, your PRP professional is able to derived a high concentration of biological nutrient-rich cells and create autologous platelet-rich plasma.

The practitioner draws a similar amount of blood required for a basic lab test.

The practitioner places the blood into a centrifuge and spins the blood at a very rapid rate to separate the platelets from the other components of the blood

The platelet- rich plasma is activated to release at least 8 essential growth factors and signaling proteins. Platelet-rich plasma is a technology that deploys aspects of blood based biochemistry. Because all constitutes retrieved in PRP are exclusively from the patient (autologous in origin), there is virtually no risk of an allergic reaction or intolerance. Nevertheless, before any treatment disclose all the medications you a retaking (including herbs) with your doctor.

Growth Factor Production Known Effects

Epidermal Growth Factor (EGF)

Plays a significant role in the regulation of cell growth, proliferation, and different iation during the remodeling phase. Stimulates keratinocyte and fibroblast production.

Interleukins, Macrophages...

...keratinocytes, endothelial cells, lymphocytes, fibroblasts, osteoblasts, basophils, mast cells: Activates fibroblast differentiation. Induces collagen and proteoglycan synthesis for healthy cell production and repair of damaged tissues.

Transforming Growth Factor (TGF)

Promotes angiogenesis which is the physiological process involving the growth of new blood vessels.

Fibroblast Growth Factor (FGF)

Promotes angiogenesis, granulation, and epithelialization for the intricate process of the skin repairing itself after injury.

Vascular Endothelial Growth Factor (VEGF)

An important signaling protein involved in both vasculogenesis and angiogenesis, the growth of blood vessels from pre-existing vasculature.

Platelet-Derived Growth Factor (PDGF)

Attracts macrophages and fibroblasts to the zone of injury. Promotes collagen growth and proteoglycan synthesis.

Collagen Stimulating Growth Factor

Stimulates granulocyte and macrophage proliferation for the growth of healthy tissue and blood cells.

Keratinocyte Growth Factor (KGF)

Keratinocyte migration, differentiation, and proliferations directly enhance wound healing and the generation of new skin.

Rhinoplasty (Nose Surgery)

What is rhinoplasty?

Rhinoplasty is a procedure that changes the size and shape of the nose, either by increasing or decreasing the bridge, changing the angle, narrowing the nostrils, or reshaping the tip. Although many seek a rhinoplasty simply to change the shape of their nose, others may choose nose surgery after a facial injury or to help relieve nasal obstructions. When having surgery to correct such a functional problem, medical insurance may cover a part or all of the surgical costs.

Rhinoplasty is usually a safe procedure with infrequent complications. When performed by a qualified cosmetic surgeon, this popular procedure can bring significant improvement in the appearance of the nose. This procedure is often one of the most gratifying procedures performed.

To ensure a successful outcome, the patient should be a good candidate, the cosmetic surgeon should be well trained in rhinoplasty techniques, and the surgeon and the patient should be very clear about what can and will be accomplished during the procedure.

Who is a Candidate for Rhinoplasty?

  • Healthy individuals with realistic expectations
  • Although there is no upper age limit for nose reshaping and nasal reconstruction, most surgeons prefer to delay surgery  on younger patients until they have completed their growth spurts – usually in the mid teens (onset of menses) for girls and the mid to late teens for boys.

Intended Results of Rhinoplasty

  • Enhance facial appearance and proportion of nose to face
  • Favorable nose size
  • Unobstructed nasal passages and ease of breathing

Procedure Description of Rhinoplasty

  • Most rhinoplasties are performed on an outpatient basis and usually take an hour or two, depending on the extent of the correction.
  • The procedure is commonly performed under a local or general anesthetic, depending on the patient and surgeon preferences.
  • Different surgeons prefer different surgical techniques. Some surgeons will use a “closed” technique, where the incisions are made inside the rim of the nostrils. Others make their incision across the base of the nostrils and nose (the “open” technique). This technique may be used in more complicated cases, because the surgeon can more easily see the surgical area.
  • In either case, the soft tissues of the nose are then separated from the underlying structures allowing access to the cartilage and bone. The cartilage and bone are then altered and sculpted into shape. At the same time, internal problems, such as breathing obstructions can be improved by removing the obstruction or changing the shape of the obstructing matter.
  • If the nostrils are too wide, the surgeon can remove a small wedge of skin from the base for a more harmonized look.

Recovery and Healing for Rhinoplasty

  • A lightweight splint may be placed on the nose for several days after to help maintain its new shape and for protection. Your surgeon may also use nasal splints inside the nose to stabilize the septum.
  • All splints, packing and stitches – if they’re not dissolved – will be removed usually by the end of one week. In some cases, splints may remain in place for several weeks, depending on the patient's condition.
  • After surgery, patients will likely experience swelling and bruising around the nose and eyes. This will peak and begin to subside after the second or third day but may last for up to 2 weeks.
  • Most patients can resume normal activities within 10 days.
  • A small amount of  bleeding may occur  during the first few days.
  • It is common to experience some nasal stuffiness, particularly if surgery was performed on the septum.
  • It may be a week or two before your surgeon will allow you to gently blow your nose.
  • Patients must be careful not to hit or bump their nose during the post operative period.
  • Sun exposure should be limited as well as it can lead to prolonged swelling.
  • Strenuous activities should be avoided for 4-6 weeks.  Elevated blood pressure during exercise can lead to additional bleeding which can significantly affect the final result.
  • It is recommended to avoid wearing glasses for a few weeks until the nose is completely healed.

→  The specific risks and the suitability of this procedure for a given individual can be determined only at the time of consultation with your cosmetic surgeon. Minor complications that do not affect the outcome occur occasionally. Major complications are rare.
